To assist in your Telehealth consultations, we have provided you ways to send digital imaging requests to The Cross Radiology. We have confirmed with the Department of Health that unsigned digital requests may be sent to us.

The above request forms can both be printed and written on. The Digital Imaging Request can be edited on your computer. Once completed, you may either print and fax it to us on 02 9579 5377 or directly email our practice (email found on top right of digital referral).
