To assist in your Telehealth consultations, we have provided you ways to send digital imaging requests to The Cross Radiology. We have confirmed with the Department of Health that unsigned digital requests may be sent to us.

The above request form can both be printed and written on or filled in online. Once completed, you may either print and fax it to us on 02 9579 5377 or directly email our practice (email found on the bottom of digital referral).
